It's time to complete the second biennial USDA Farm to School Census!

Back in 2013 we learned that more than 40,000 schools, serving 23.5 million students, were "bringing the farm to school." A lot can change in two years and we're excited to receive updated information through the 2015 USDA Farm to School Census.

Please complete the questionnaire even if you are not currently purchasing any locally produced food or participating in farm to school by May 15 2015. Please complete a separate Farm to School Census questionnaire for each school district SFA you manage.

If you would rather complete the questionnaire on paper,please print out a copy of the questionnaire, fill it out by hand and submit it via fax to (609) 9361462.  If you need assistance completing the online or paper copy, please call the 2015 Farm to School Census help line at 1(844) 218‐5330.

The 2015 Farm to School Census will provide updated information about how many school districts are purchasing locally produced food for school meal programs or conducting other types of farm to school activities. This information collection is authorized under 7 U.S.C. 427 and approved by the Office of Management and Budget (OMB Control No. 0584-0593, expires February 29, 2016).

The questionnaire does not include any personal information. However, we must inform you that your responses are not confidential and are not covered by the Confidential Information Protection and Statistical Efficiency Act of 2002. USDA will treat all information gathered in accordance with the Freedom of Information Act (5 U.S.C. 552).
